ABSTRACT

ABSTRACT Background: Colorectal cancer is one of the most prevalent pathologies worldwide whose prognosis is linked to early detection. Colonoscopy is the gold standard for screening, and diagnosis is usually made histologically from biopsies. Aiming to reduce the inspection and diagnostic time as well as the biopsies and resources involved, other techniques are being promoted to conduct accurate in vivo colonoscopy assessments. Optical biopsy aims to detect normal and neoplastic tissues analysing the autofluorescence spectrum based on the changes in the distribution and concentration of autofluorescent molecules caused by colorectal cancer. Therefore, the autofluorescence contribution analysed by image processing techniques could be an approach to a faster characterization of the target tissue. Objective: Quantify intensity parameters through digital processing of two data sets of three-dimensional widefield autofluorescence microscopy images, acquired by fresh colon tissue samples from a colorectal cancer murine model. Additionally, analyse the autofluorescence data to provide a characterization over a volume of approximately 50 µm of the colon mucosa for each image, at second (2nd), fourth (4th) and eighth (8th) weeks after colorectal cancer induction. Methods: Development of a colorectal cancer murine model using azoxymethane/dextran sodium sulphate induction, and data sets acquisition of Z-stack images by widefield autofluorescence microscopy, from control and colorectal cancer induced animals. Pre-processing steps of intensity value adjustments followed by quantification and characterization procedures using image processing workflow automation by Fiji's macros, and statistical data analysis. Results: The effectiveness of the colorectal cancer induction model was corroborated by a histological assessment to correlate and validate the link between histological and autofluorescence changes. The image digital processing methodology proposed was then performed on the three-dimensional images from control mice and from the 2nd, 4th, and 8th weeks after colorectal cancer chemical induction, for each data set. Statistical analyses found significant differences in the mean, standard deviation, and minimum parameters between control samples and those of the 2nd week after induction with respect to the 4th week of the first experimental study. This suggests that the characteristics of colorectal cancer can be detected after the 2nd week post-induction. Conclusion: The use of autofluorescence still exhibits levels of variability that prevent greater systematization of the data obtained during the progression of colorectal cancer. However, these preliminary outcomes could be considered an approach to the three-dimensional characterization of the autofluorescence of colorectal tissue, describing the autofluorescence features of samples coming from dysplasia to colorectal cancer.


RESUMO Contexto: O câncer colorretal é uma das patologias mais prevalentes em todo o mundo, cujo prognóstico está ligado à detecção precoce. A colonoscopia é o padrão ouro para triagem, e o diagnóstico geralmente é feito histologicamente a partir de biópsias. Visando reduzir o tempo de inspeção e diagnóstico, bem como as biópsias e recursos envolvidos, outras técnicas estão sendo promovidas para realizar avaliações precisas de colonoscopia in vivo. A biópsia óptica visa detectar tecidos normais e neoplásicos analisando o espectro de autofluorescência com base nas mudanças na distribuição e concentração de moléculas autofluorescentes causadas pelo câncer colorretal. Portanto, a contribuição da autofluorescência analisada por técnicas de processamento de imagem poderia ser uma abordagem para uma caracterização mais rápida do tecido-alvo. Objetivo: Quantificar parâmetros de intensidade por meio do processamento digital de dois conjuntos de dados de imagens de microscopia de autofluorescência em campo amplo tridimensionais, adquiridas por amostras de tecido fresco de cólon de um modelo murino de câncer colorretal. Adicionalmente, analisar os dados de autofluorescência para fornecer uma caracterização em um volume de aproximadamente 50 µm da mucosa do cólon para cada imagem, na segunda (2ª), quarta (4ª) e oitava (8ª) semanas após a indução do câncer colorretal. Método: Desenvolvimento de um modelo murino de câncer colorretal usando indução de azoximetano/sulfato de sódio dextrano, e aquisição de conjuntos de dados de imagens Z-stack por microscopia de autofluorescência em campo amplo, de animais controle e induzidos ao câncer colorretal. Etapas de pré-processamento de ajustes de valores de intensidade seguidas por procedimentos de quantificação e caracterização usando automação de fluxo de trabalho de processamento de imagem por macros do Fiji, e análise estatística de dados. Resultados: A eficácia do modelo de indução de câncer colorretal foi corroborada por uma avaliação histológica para correlacionar e validar a ligação entre as mudanças histológicas e de autofluorescência. A metodologia de processamento digital de imagem proposta foi então realizada nas imagens tridimensionais de camundongos controle e das 2ª, 4ª e 8ª semanas após a indução química do câncer colorretal, para cada conjunto de dados. Análises estatísticas encontraram diferenças significativas nos parâmetros médios, desvio padrão e mínimos entre amostras de controle e aquelas da 2ª semana após a indução em relação à 4ª semana do primeiro estudo experimental. Isso sugere que as características do câncer colorretal podem ser detectadas após a 2ª semana pós-indução. Conclusão: O uso de autofluorescência ainda apresenta níveis de variabilidade que impedem uma maior sistematização dos dados obtidos durante a progressão do câncer colorretal. No entanto, esses resultados preliminares podem ser considerados uma abordagem para a caracterização tridimensional da autofluorescência do tecido colorretal, descrevendo as características de autofluorescência de amostras que vão da displasia ao câncer colorretal.

ABSTRACT

Purpose: To describe the phenotypic variations in family members of patients with retinitis pigmentosa (RP) with different modes of inheritance and to assess the ocular abnormalities in RP families. Methods: A descriptive analysis of three types of inheritance of RP was carried out, where 64 family members were examined at a tertiary eye care center, South India. They underwent comprehensive eye examination, fundus photography, fundus autofluorescence (FAF), full?field electroretinogram (FFERG), and spectral domain optical coherence tomography (SD?OCT). Analysis was performed between mild and severe forms of abnormalities to delineate retinal structural and functional defects in RP families. Results: The mean age was 38.55 ± 17.95 years. Males were 48.4%. In autosomal recessive and X?linked recessive groups, 74.2% and 77.3%, respectively, were asymptomatic, whereas in autosomal dominant group, 27.3% were asymptomatic. The proportion of the cases with abnormalities in all three groups was higher on ERG (59.6%), followed by OCT (57.5%), visual acuity (43.7%), peripheral FAF (23.5%), and macular FAF (11.8%). However, these abnormalities and the clinical pictures of the family members had no statistical difference across the three groups of inheritance. Conclusion: Structural and functional retinal alterations were noted in four out of five asymptomatic members, suggesting the need for careful screening of RP families and the pressing need for pre?test (genetic) counseling

ABSTRACT

Fundus autofluorescence(FAF)imaging is based on the fluorescence excited by the endogenous fluorophores in the retinal pigment epithelium and choroid, mainly referring to lipofuscin(LF)and melanin. The non-invasive examination method can imply the function of retinal pigment epithelium by observing the spatial distribution of LF or melanin. This technique has unique advantages in the diagnosis and differentiation of age-related macular degeneration(ARMD). This guideline is to standardize and interpret the application of FAF in different stages and classifications of ARMD.

ABSTRACT

Objective:To observe the application of near-infrared autofluorescence imaging (NIRAF) technology combined with carbon nanoparticle (CNP) negative imaging in identification of parathyroid gland (PG) during thyroid carcinoma surgery.Methods:80 patients with thyroid cancer who underwent total thyroidectomy + central lymph node dissection performed by the same experienced physician team at the 960th Hospital of the PLA from Jan. to Mar. 2022 were prospectively included. Before operation, they were divided into two groups using random number table method before surgery: control group (40 cases) using CNP negative imaging, and experimental group (40 cases) using CNP negative imaging combined with NIRAF technique for intraoperative identification of PG. The gold standard for the identification of parathyroid glands was to compare the amount of intraoperative discovery retention misresection and transplantation of PG and the number of postoperative parathyroid hormone (PTH) and the number of complications in the two groups by immune colloidal gold technique. SPSS 25.0 software was used for statistical analysis.Results:All patients in the two groups were successfully operated and followed up. 137 149 PG were found and confirmed in the control group and the observation group, 108 132 PG were retained in situ and 29 17 PG were transplanted, the differences were statistically significant (all P <0.05) ; The number of A1 PG was 103 and 109, respectively. Among them, 84 102 were retained in situ and 19 7 were transplanted, the difference was statistically significant ( P <0.05) . There was no significant difference in the amount of A2 type PG and B type PG between the two groups ( P >0.05) . No A3 type PG was found in the two groups, and a total of 3 A3 types of PG were confirmed in postoperative pathological reports. There were no significant differences in misresection in the control group and the observation group, 5 and 2 PG were mistakenly cut, respectively (all P >0.05) . The PTH 1 day after surgery was 17.7 (5.6,30.4) pg/mL in the control group and 21.7 (12.8,38.3) pg/mL in the observation group, the difference was statistically significant ( P<0.05) . There were no significant differences in the levels of serum calcium and serum phosphorus 1 day after operation and PTH 1 month after surgery between the two groups (all P > 0.05) . Conclusion:Compared with CNP alone, combined with NIRAF technique can quickly and effectively identify PG, and PG can be better protected in situ and postoperative hypoparathyroidism can be reduced.

Objective:To evaluate the excitation and identification of parathyroid autofluorescence in thyroid surgery by a fluorescence laparoscopic system.Methods:The clinical data of 6 patients with papillary thyroid carcinoma who underwent Laparoscopic thyroid surgery at the Second Affiliated Hospital of Guangxi Medical University from Mar. 2022 to Apr. 2022 were collected. The autofluorescence of the parathyroid glands was detected and identified by the OptoMedic fluorescence laparoscopic system during the operation. A rapid frozen sections pathological examination of possible parathyroid tissue with autofluorescence was performed to determine whether it was parathyroid tissue.Results:Parathyroid autofluorescence with different intensities was visualized intraoperatively in all 6 patients, and it was confirmed as parathyroid tissue by rapid frozen pathological section.Conclusions:The parathyroid gland has a unique autofluorescence feature. Using this feature to identify, locate and protect the parathyroid gland during surgery can help reduce the complications of parathyroid injury.

ABSTRACT

Purpose: To assess the autofluorescence size and properties of pterygium and pinguecula by anterior segment autofluorescence (AS-AF) imaging and demonstrate the difference of autofluorescence size presented in AS-AF imaging compared to the extend size of the conjunctival lesion measured by anterior segment slit-lamp photography (AS-SLE). Methods: Twenty-five patients with primary pterygium and twenty-five with pinguecula were included in the study. In addition, 25 normal subjects were also enrolled as the control group. The AS-AF characteristics of pterygium and pinguecula lesions were analyzed. The size of lesions displayed in the AS-SLE photography versus the AS-AF images were also compared. AS-AF images were obtained using a Heidelberg retina angiograph which focused on the anterior segment. AS-SLE photography was acquired using a digital imaging system (BX900 HAAG STREIT). Results: There were 44 (58.7%) male and 31 (41.3%) female patients; 19 (76%) and 20 (80%) patients had bilateral pterygium and pinguecula, respectively. All pinguecula lesions reflected hyperautofluorescence pattern in the AS-AF imaging. In 24 (96%) patients, the hyperautofluoresecence pattern was larger than the size of the clinical lesions displayed with the AS-SLE photography. Twenty-one (84%) patients with pterygium reflected a hyperautofluorescence pattern in AS-AF images; in one (4%) patient, the hyperautofluorescence pattern was larger than the clinical lesion size and four (16%) patients had no autofluorescence patterns in the AS-AF images. In the control group, in 14 (56%) subjects, a hypoautofluorescent pattern was revealed in the conjunctiva in AS-AF images. However, in 11 (44%) patients, hyperautofluorescence patterns were detected. Conclusion: AS-AF is a useful modality to monitor vascularization in conjunctival lesions. Pingueculae and pterygium show hyperautofluorescence in AS-AF imaging. The real size of the pinguecula lesions may be estimated with AS-AF characteristics, mostly presenting larger than the area size in AS-SLE photography. The autofluorescence size of the pterygium is smaller than the extent of visible pterygium in slit-lamp photography.

Abstract Introduction: Traditional meta-analyses on the diagnostic accuracy of oral lesions have been conducted, but they were inherently limited to direct pairwise comparisons between a single method and a single alternative, while multiple diagnostic options and the ranking thereof were methodologically not possible. Objective: To evaluate the diagnostic values of various methods in patients with oral potential malignant disease by performing a network meta-analysis. Methods: Two authors independently searched the databases (MEDLINE, SCOPUS, the Cochrane Register of Controlled Trials, and Google scholar) up to June 2020 for studies comparing the diagnostic accuracy of various tools (autofluorescence, chemiluminescence, cytology, narrow band imaging, and toluidine blue) with visual examination or other tools. The outcomes of interest for this analysis were sensitivity, specificity, negative predictive value, positive predictive value and accuracy. Both a standard pairwise meta-analysis and network meta-analysis were conducted. Results: Treatment networks consisting of six interventions were defined for the network metaanalysis. The results of traditional meta-analysis showed that, among six methods, narrow band imaging showed higher sensitivity, specificity, negative predictive value, positive predictive value, and accuracy compared to visual examination. The results of network meta-analysis showed that autofluorescence, chemiluminescence, and narrow band imaging had higher sensitivity compared with visual examination, and that chemiluminescence and narrow band imaging had higher negative predictive value compared with visual examination. However, autofluorescence and chemiluminescence had lower specificity compared with visual examination. There were no significant differences in positive predictive value and accuracy among the six interventions. Conclusion: This study demonstrated that narrow banding imaging has superiority in terms of sensitivity and negative predictive value compared with the other five tested agents.


Resumo Introdução: Metanálises tradicionais sobre a precisão diagnóstica de lesões orais têm sido conduzidas, mas são inerentemente limitadas a comparações pareadas diretas entre um único método e uma única opção, enquanto múltiplas opções de diagnóstico e suas classificações ainda não foram metodologicamente possíveis. Objetivo: Avaliar os valores diagnósticos de vários métodos em pacientes com doença oral potencialmente maligna e fazer uma metanálise de rede. Método: Dois autores pesquisaram independentemente os bancos de dados (Medline, Scopus, Cochrane Register of Controlled Trials e Google Scholar) até junho de 2020 para estudos que comparassem a precisão diagnóstica de várias ferramentas (autofluorescência, quimioluminescência, citologia, imagem de banda estreita e cloreto de tolônio) com exame visual ou outras ferramentas. Os resultados de interesse para esta análise foram sensibilidade, especificidade, valor preditivo negativo, valor preditivo e precisão. Tanto uma metanálise pareada padrão quanto uma metanálise de rede foram conduzidas. Resultados: Redes de tratamento compostas por seis intervenções foram definidas para a metanálise de rede. Os resultados da metanálise tradicional mostraram que, entre seis métodos, a imagem de banda estreita apresentou maior sensibilidade, especificidade, valor preditivo negativo, valor preditivo e precisão em comparação ao exame visual. Os resultados da metanálise de rede mostraram que a autofluorescência, a quimioluminescência e a imagem de banda estreita obtiveram maior sensibilidade em comparação com o exame visual e que a quimioluminescência e a imagem de banda estreita apresentaram maior valor preditivo negativo em comparação com o exame visual. Entretanto, a autofluorescência e a quimioluminescência mostraram especificidade inferior em comparação com o exame visual. Não houve diferenças significativas no valor preditivo e na precisão entre as seis intervenções. Conclusão: Este estudo demonstrou que a imagem de banda estreita demonstra superioridade em termos de sensibilidade e valor preditivo negativo em comparação com os outros cinco agentes testados.

@#AIM: To explore the characteristics and application value of ultra-wide-field auto-fluorescence in Stargardt disease. <p>METHODS: A retrospective study of clinical characteristics and genetically confirmed Stargardt disease patients, using Optos P200Tx for ultra-wide-field auto-fluorescence imaging, and comparing the imaging features of traditional fundus color photographs, fundus 55° auto-fluorescence, and optical coherence tomography, to evaluate the ultra-wide-field auto-fluorescence in Stargardt disease image characteristics and advantages of clinical application. <p>RESULTS:All 76 eyes(100%)had high posterior auto-fluorescence, while only 42 eyes(55%)of traditional 55° auto-fluorescence showed obvious posterior high auto-fluorescence. Sixty-six eyes(87%)of the 76 eyes showed different numbers of spots, which were distributed from the posterior pole to the peripheral retina. UWAF can show clearer and more number of retinal spots than fundus color photographs, and more completely shows the number and distribution of spots area. All 76 eyes(100%)of the patients showed the oval low auto-fluorescence area induced by retinal pigment epithelium(RPE)atrophy in the center of the macula. As the disease progressed, the atrophy area expanded and the low fluorescence area expanded accordingly. Ultra-wide-field auto-fluorescence can completely display the atrophy range and area, but it cannot display in dystrophy depth. In 48 eyes(63%)ultra-wide-field auto-fluorescence, strong background auto-fluorescence was seen extending from the macula to the nasal and inferior temporal of the optic disc, forming a clear approximately vertical dividing line below the optic disc.<p>CONCLUSION: Ultra-wide-field auto-fluorescence changes in Stargardt are not limited to the posterior pole and may extend more peripherally. Ultra-wide-field imaging is a useful tool for the assessment of patients with Stargardt macular dystrophy.

OBJECTIVE@#To evaluate the value of fundus autofluorescence (FAF) imaging combined with spectral domain optical coherence tomography (SD-OCT) in diagnosis, prognostic assessment and follow-up observation of acute Vogt-KoyanagiHarada (VKH) disease.@*METHODS@#Clinical data were collected from 12 patients (23 eyes) with acute VKH disease treated in our hospital from May, 2018 to November, 2019, including detailed medical history, best corrected visual acuity (BCVA), and results of slit lamp biomicroscopy, fundus photography, SD-OCT, fundus fluorescein angiography (FFA) and FAF imaging.SDOCT and FAF imaging were repeated after a course of treatment and in follow-up examination, and the results were compared with those at the time of admission.@*RESULTS@#VKH disease involved both eyes in 11 patients (91.7%).Fundus photography showed optic disc edema in 16 eyes (69.6%), and multiple retinal neuroepithelial detachment was detected by SD-OCT in all the involved eyes (100%).IN all the eyes, FFA revealed small and dense fluorescein leakage in the early stage and fluorescein accumulation in advanced stages of VHK disease to form multiple dye pooling in the areas of serous detachment.Hyperauto fluorescence was a common finding in FAF imaging (100%), and the area involved was consistent with that of fluorescein accumulation shown by FAF imaging.Ten eyes (43.5%) showed patches of relative hypoautofluorescence in the hyperauto fl uorescence areas, and granular hyperauto fl uorescence was found in the lesions in 4 eyes (17.4%).During the remission period of VKH disease, FAF imaging showed normal finding in 8 eyes (34.8%) and reduced areas (by 55.2%) and intensity (by 46.5%) of hyperautofluorescence in 9 eyes (39.1%).In 6 eyes (26.1%), only a few hyperautofluorescent spots scattered in the macula were observed.SD-OCT demonstrated significantly reduced (by 69.5% on average) or even disappearance of subretinal fluid in the eyes.The fluorescence intensity in FAF imaging showed a significant positive correlation with the volume of subretinal fluid detected by SD-OCT (@*CONCLUSIONS@#The combination of fluorescein angiography, FAF imaging and SD-OCT can significantly improve the diagnostic accuracy of VKH disease.FAF imaging combined with SD-OCT provides an effective and noninvasive modality for evaluation of remission and monitoring the changes in VKH disease.

Objective: The condition of the resected margin in oral squamous cell carcinoma continues to be an important prognostic factor; the use of optic technology could help surgeons in determining the margin status at real time. This study aims to evaluate Oral ID, a hand held device that uses the principal of auto-fluorescence to determine surgical safe margins in patients with oral squamous cell carcinoma, and to compare the results with those of the conventional 1 cm margin method. Material and Methods: This study was a descriptive, comparative analytical study carried out at Khartoum Dental Teaching Hospital and Oral Histopathology Diagnostic Laboratory, Faculty of Dentistry, University of Khartoum. A total of 92 margins obtained from 31 patients, 46 margins were taken by Oral ID and the other 46 were taken by the traditional 1cm method. All margins were examined histologically with conventional Hematoxylin and Eosin stain. Results: It was found that all tumors showed fluorescence loss; A significant association was found between the use of Oral ID and obtaining a free margin P (0.02) the sensitivity of Oral ID was found to be 74% the specificity was found to be 89%. Ten out of the 46 margins obtained by fluorescence showed mild dysplasia and two margins showed high grade dysplasia. The 46 margins obtained by the traditional 1cm margin showed different field alterations two were involved, one was close, five showed high grade dysplasia and 14 showed mild dysplasia yielding a specificity of 52.2%. Conclusion: Using Oral ID for surgical margin assessment increases the accuracy to 74% compared to the conventional method which was found to be 52.2%. The results of the device are comparable to other auto-fluorescence devices of different trademarks. Further development of the device to help overcome its limitations is strongly advised (AU)


Objetivo: A condição da margem ressecada no carcinoma oral de células escamosas continua sendo um importante fator prognóstico; o uso de tecnologia óptica pode ajudar cirurgiões a determinar o status da margem em tempo real. O objetivo deste estudo é avaliar o Oral ID, um aparelho portátil que utiliza o princípio da autofluorescência para determinar margens de segurança cirúrgicas em pacientes com carcinoma oral de células escamosas, e comparar os resultados com o método convencional de margem de 1 cm. Material e Métodos: Este estudo foi um estudo descritivo, analítico e comparativo realizado no Khartoum Dental Teaching Hospital e no Laboratório de Diagnóstico de Histopatologia Oral da Faculdade de Odontologia, Universidade de Khartoum. Um total de 92 margens foram obtidas de 31 pacientes, 46 margens foram obtidas por Oral ID e as outras 46 foram obtidas pelo método tradicional de 1 cm. Todas as margens foram examinadas histologicamente com coloração convencional de Hematoxilina e Eosina. Resultados: Verificou-se que todos os tumores apresentaram perda de fluorescência; uma associação significativa foi encontrada entre o uso de Oral ID e a obtenção de uma margem livre P (0,02), a sensibilidade de Oral ID foi de 74% e a especificidade de 89%. Dez das 46 margens obtidas por fluorescência mostraram displasia leve e duas margens mostraram displasia de alto grau. As 46 margens obtidas pela margem tradicional de 1cm apresentaram diferentes alterações de campo, duas estavam envolvidas, uma estava próxima, cinco apresentaram displasia de alto grau e 14 apresentaram displasia leve com especificidade de 52,2%. Conclusão: O uso de Oral ID para avaliação da margem cirúrgica aumenta a acurácia para 74% em comparação com o método convencional, que foi encontrado em 52,2%. Os resultados do dispositivo são comparáveis a outros dispositivos de autofluorescência de diferentes marcas comerciais. O desenvolvimento do dispositivo para ajudar a superar suas limitações é fortemente recomendado. (AU)

ABSTRACT

Purpose: The aim of this study is to assess the ability of multicolour imaging (MCI) to detect foveal cysts in diabetic macular edema (DME) and compare it with conventional color fundus photography (CFP) and foveal autofluorescence (FAF) pattern. Methods: It was a retrospective review of 112 eyes of 84 DME patients with central foveal thickness ?250 ? who underwent MCI, CFP and shortwave autofluorescence imaging. MCI was performed with Sepctralis spectral domain optical coherence tomography (SDOCT) (Heidelberg Engineering, Germany). Results: 97 (86.6%) eyes had cystoid increased autofluorescence (cystoid iFAF), 9 (8%) had spot iFAF and 6 (5.35%) had irregular decreased FAF (dFAF). Among eyes with cystoid iFAF, OCT detected DME cysts in 93 (95.6%) eyes, MCI in 75 (77.3%) and CFP in 5 (5.15%) eyes. In all these eyes, the location of cysts on OCT and MCI corresponded with the location of cystoid iFAF, whereas none of the eyes with cyst seen on CFP correlated with the location of cystoid iFAF. Conclusion: MCI was superior to CFP in detecting DME cysts at fovea. It also correlated with hyperautofluorescence pattern in these eyes. MCI may have a potential role in diabetic retinopathy screening by segregating eyes with DME which would require treatment. Our findings need to be further validated in a larger and prospective study design.

ABSTRACT

Purpose: To evaluate the potential clinical utility of wide-field swept source optical coherence tomography angiography (SS-OCTA) using a prototype device compared to a wide-field fundus autofluorescence (FAF) for analysis of the disease activity in eyes with tubercular serpiginous-like choroiditis (TBSLC). Methods: Using a prototype SS-OCTA device (PLEX Elite, Carl Zeiss Meditec, Dublin, CA), 17 eyes of 12 consecutive patients with TBSLC were imaged and multiple 12 mm � 12 mm OCTA scans were captured, which were montaged to create wide-field montage OCTA images scans. A wide- FAF (Eidon, CenterVue, Padova, Italy) was performed in the same sitting. Two masked graders independently analyzed OCTA and FAF images for the presence of choroidal lesions, recorded the number of lesions identifiable, and provided a subjective grading for the activity of individual lesion, which were then compared. Results: The total number of lesions identified on FAF were 282 (posterior pole lesions, n = 129 and peripheral lesions n = 153) and on wide-field SS-OCTA were 230 (posterior pole lesions, n = 108 and peripheral lesions n = 122). Active choroidal lesions were comparable on the two machines (n = 28 on FAF and n = 28 on SS-OCTA, respectively); whereas numerous healed lesions were identified on FAF (n = 219) as compared to SS-OCTA (n = 170). There was good correlation among the devices for healed lesions (Pearson correlation, r = 0.82) and active lesions (r = 0.88). Conclusion: There was good correlation between FAF and wide-field SS-OCTA for detection of disease activity in TBSLC; however, FAF depicted greater number of healed lesions compared to wide-field SS-OCTA.

ABSTRACT

Objective To compare different blocking methods for eliminating autofluorescence of mouse liver frozen sections, so as to find the best method to reduce the interference to immunofluorescence positive signals and improve the accuracy of immunofluorescence. Methods Intrasplenic injection-liver colonization nude mice: Hepa1-6-GFP cells were intrasplenically injected into male athymic BALB/c nude mice to create liver colonization models. Liver tissues were frozen and continuously sectioned. Sections were blocked with AB reagent (A reagent: streptavidin reagent, B reagent: biotin reagent), blocking buffer, AB reagent+blocking buffer, or acetone+AB reagent+blocking buffer, and then the autofluorescence of the frozen sections was detected. C57BL/6 mice: the liver tissues of C57BL/6 mice were frozen and continuously sectioned, and then the sections were blocked with AB reagent, blocking buffer, AB reagent+blocking buffer, or acetone+AB reagent+blocking buffer. Liver macrophages (Kupffer cells) were labeled with F4/80, and then the autofluorescence of mouse liver frozen sections was detected. Results In the immunofluorescence staining of liver tissue frozen sections, all the above four blocking methods could reduce the autofluorescence of liver sections, and acetone+AB reagent+blocking buffer group had the best effect. Conclusion The combined buffers (acetone+AB reagent+blocking buffer) has the best effect in eliminating the autofluorescence of mouse liver frozen section.

?AIM:To analyze the morphological changes of macular soft drusen and drusenoid pigmental epithulium detachment ( DPED ) after subthreshold micropulse laser treatment ( SMLT) .?METHODS: Fourteen patients ( 20 affected eyes ) with soft drusen and DPED clinically confirmed from August 2016 to October 2018, were included in this study. 577 nm yellow laser of SMLT was applied for soft drusen and DPED. The changes of soft drusen and DPED in best corrected visual acuity ( BCVA ) ( LogMAR ) and height, diameter and cross-sessional area according to fundus autofluorescence and SD - OCT examinations were observed after SMLT.?RESULTS: BCVA was not significant difference after treatment of soft drusen (P=0.260), and the DPED (P=0. 736 ) than that of the baseline. Compared with the baseline values, the height and cross-sessional area of soft drusen were reduced at the 6mo after treatment ( P=0. 008, P=0.034) . Compared with the baseline values, the differences were not statistically significant in height, diameter and cross - sectional area of DPED after treatment.?CONCLUSION: BCVA was not reduced for drusen and DPED after SMLT, however, the height and cross -sessional area of soft drusen was reduced compared with those before treatment, and the differences were not statistically significant in height, diameter and cross -sectional area of DPED before and after treatment. @#AIM:To observe the clinical effects of YAG laser ablation in patients with physiological vitreous floaters by A/B ultrasound combined with autofluorescence(AF)of angiography.<p>METHODS: Screening of 30 patients(30 eyes)with physiological vitreous floaters from August 2018 to January 2019 in Hankou Aier Eye Hospital, we used A/B ultrasound to measure the distance from the wall of the ball and the back surface of the lens before operations. Check the patient's best corrected visual acuity, non-contact intraocular pressure, scanning laser fundus photography, A/B ultrasound and autofluorescence(AF)of angiography before treatment, laser ablation was performed by the same skilled doctor using ELLEX YAG laser ablation machine. After treatment, the patients were followed up at the point of 1d, 1wk, and 1mo. The same examination was performed after operation. The Image-J software was used to measure the vitreous opacity in the autofluorescence of angiography pictures before and after treatment.<p>RESULTS:There was no significant difference in the best corrected visual acuity and non-contact intraocular pressure before and after treatment(<i>P</i>>0.05). Of the 30 eyes, 28 eyes underwent laser ablation treatment once, and 2 eyes underwent laser ablation treatment twice. The average number of treatments was 1.06. After treatment, the autofluorescence of angiography showed that the vitreous opacity was obviously reduced or disappeared, and the difference was statistically significant before and after treatment(<i>P</i><0.01). No complications occurred during or after surgery.<p>CONCLUSION: A/B ultrasound combined with autofluorescence confirmed that Nd:YAG laser is safe and effective in the treatment of physiological vitreous floaters.

ABSTRACT

@#AIM: To explore the multimodal imaging features of choroidal osteoma.<p>METHODS:Retrospectively observational case series. Nine patients(15 eyes)with choroidal osteoma presented to Xi'an No.3 hospital from October 2015 to August 2019 were included in the study. Among them, there were 2 males and 7 females. All patients received the best corrected visual acuity, fundus color photography, short wavelength fundus autofluorescence(SW-AF), infrared fundus autofluorescence(IR-AF), fundus fluorescein angiography(FFA), indocyanine green angiography(ICGA), optical coherence tomography(OCT)and orbital X-ray computed tomography(CT). <p>RESULTS: In 15 affected eyes, lesions were involved in macula and optic disc in 5 eyes(33%), lesions were located in macula in 8 eyes(53%), peripapillar lesions in 2 eyes(13%). Choroidal osteomas presented as orange-red or yellow-white lesions with pigmentation on the surface in fundus color photographies. SW-AF images showed that choroidal osteomas presenting with mottled hypofluorescence and hyperfluorescence in 15 eyes(100%), while IR-AF images showed that all of the choroidal osteomas presenting punctate and patchy hypofluorescence and hyperfluorescence. FFA showed that 15 eyes(100%)of choroidal osteoma had gradually enhanced brightness, of which 6 eyes(40%)had severe leakage of fluorescence from sub-retinal neovascularization(SRNV). ICGA images revealed SRNV in 9 eyes(60%), all corresponding area of choroidal osteomas showed low fluorescence in the early stage of angiography, and the brightness of tumors increased during angiography gradually. OCT images showed that choroidal osteomas with inhomogeneous reflection in 10 eyes(67%); choroidal osteomas presented with hypo-reflectivity in 5 eyess(33%). All of the choroidal osteomas(100%)presented as high density bone mass on CT images.<p>CONCLUSION: Choroidal osteoma is orange-red or yellow-white, and expressed as bone mass on CT images, which is an important basis for diagnosis of choroidal osteoma. Damaged RPE leads to appearance of strong and weak mottled SW-AF and IR-AF in the location of choroidal osteomas. FFA and ICGA are helpful to clarify the abnormal circulation in the lesion area. The intensity of cross-sectional reflection in choroidal osteomas are different on OCT images, meanwhile, OCT was helpful to observe the condition of subretinal effusion and neovascularization.

ABSTRACT

A 28-year-old male presented to retina clinic with complains of blurring and distortion in right eye for past 1 week. There was history of implantation of phakic intraocular lens (pIOL) bilaterally 4 months back. Ophthalmic examination revealed a full-thickness macular hole in the right eye. Pars plana vitrectomy with inverted internal limiting membrane flap was planned. Post-operatively, patient had a good gain in vision (20/40) with closure of the hole. Macular hole is an unusual complication of pIOL. A detailed pre-operative fundus screening is indispensable. Early presentation and timely intervention can optimize the visual outcome.
